Do I qualify?

MAP provides health care coverage for Travis County residents with low income, who are not eligible for or enrolled in Medicaid or Medicare and do not have private insurance

Is MAP for me?

You must live in Travis County to qualify for MAP.
To get more information or find out if you qualify call Central Health Customer Service at 512-978-8130 Monday-Friday between 8:00am and 5:00pm.
A Customer Service representative will need to ask you some questions to determine if you qualify for MAP.
Some of the things you may be asked about are:
-Address
-Family size
-Income
-Insurance

Important

MAP and MAP BASIC are not federal programs therefore are not impacted by the new “Public Charge” regulation the Federal Government has proposed. The new Public Charge regulation is not expected to take effect until Oct. Central Health is monitoring the changes and will provide more information as it is available.

All uninsured Travis County uninsured residents eligible for MAP and MAP BASIC should continue to enroll without fear of impacting their path to a green card.

Income Requirements

To qualify for full benefits under the Medical Access Program (MAP), you must be at or below 100% of the Federal Poverty Level (FPL). Elderly or disabled residents must be at or below 200% FPL.

If you are at or below 200% FPL who do not qualify for MAP full-benefit coverage may qualify for MAP BASIC, which provides a more limited level of services.

You might also qualify for primary care services through our health care partners on a sliding fee scale program that determines the cost for care based on annual income.

  • $12,760 for individuals
  • $17,240 for a family of 2
  • $21,720 for a family of 3
  • $26,200 for a family of 4
  • $30,680 for a family of 5
  • $35,160 for a family of 6
  • $39,640 for a family of 7
  • $44,120 for a family of 8

How To Apply for MAP

As a result of containment measures being taken to minimize spread of the novel coronavirus (COVID-19), many Travis County residents are losing their jobs, their income, and their health insurance. If you are uninsured and have lost your job, you may be eligible for MAP or MAP BASIC.
If you apply online you will be able to upload your documents in your application.
  1. Online: apply4map.net
  2. Over the phone by calling 512-978-8130
  3. Download a paper application, fill it out, and mail it in:
    • English
    • Spanish
  4. Make an appointment by calling 512-978-8130 and apply in person (NOTE: Due to COVID-19 all MAP Eligibility offices are closed)

If you have already applied and received a letter from us requesting you provide us with additional information or documents, you can submit those documents:

  • By Fax at 512-776-0457 or
  • By email [email protected] (Note: applications and documents sent through a client’s email may not be secure)

Required Documents to Submit with Your Application

Proof that you live in Travis County.
Examples include:
– Current Texas driver’s license
– Utility bill
– Mail postmarked in the last 30 days
Any proof of U.S. citizenship or residency for your household members.
Examples include:
– Birth certificate
– Legal Permanent Resident Card
– Passport
– Other USCIS documents
Picture ID for all adults in your household.
Examples include:
– State-issued driver’s license
– Foreign ID
– USCIS document
– Passport
Proof you are the parent or legal guardian of all children in your household.
Examples include:
– Birth certificate
– Adoption papers
– Legal guardianship papers
– Court documents
Proof of any other health care benefits for which you have been approved, been denied, or have a pending application.
Examples include:
– Medicaid enrollment verification letter
– Medicare card
– Private insurance card
– Denial letter for Medicaid or CHIP
– Letter from Social Security Administration indicating pending application
– Letter indicating pending application for TANF / Medicaid
Proof of your household’s income.
Examples include:
-Check stubs
– Cash contribution letter
– Retirement or Social Security benefits letter
– Unemployment benefits letter
– Child support payment letter
– Self-Employment Statement
– MAP’s Residence and/or Financial Support Statement
– MAP’s Employment Verification Form
Other forms
– Applicant Responsibilities
