- Go to the online application (link) and select English or Spanish to begin.
- The online application will let you upload your required documents. If you are missing some information or documents, you will receive a letter on what is needed with instructions on how to send it. Someone from our eligibility team will also try and call you.
- When you are enrolled in MAP or MAP BASIC, we will mail you your card and handbook. If you aren’t eligible for MAP we will send you information on other health coverage programs.
Apply Over The Phone
- Before you start, get together the needed documents and information. (insert link to documents page)
- Call 512-978-8130 to talk to an Eligibility Specialist who will explain how to apply.
- They can complete an application over the phone and mail it to you to sign and send back. They will also send a list of the documents you will need to send with your application.
- When you are enrolled in MAP or MAP BASIC, we will mail you your card and handbook. If you aren’t eligible for MAP we will send you information on other health coverage programs.
Don't let your MAP coverage expire!
It’s important you can see your doctor for regular check-ups or if you get sick.
- Look at your card – you will see an expiration date in the corner.
- Set a reminder to renew your MAP card at least 30 days before that date.
- It’s easy – just call or go online and resubmit an application. You’ll receive your new card in the mail.
